OvaFlow

Clinical authority

Staff decide. OvaFlow never prescribes.

OvaFlow coordinates around clinical decisions. Every decision, signature and lab determination stays with the qualified person and the clinic's own systems.

Examples use fictional patient data

The principle

Two sides of one line

Everything OvaFlow does sits on its side of the line and stops there. Everything on the other side belongs to a qualified person, in the clinic's own systems.

OvaFlow

  • Prepares drafts from the signed source
  • Tracks who owns each next step
  • Checks the EMR before work closes
  • Holds the action when something is unknown

Your clinicians and staff

  • PhysicianPrescribe, and choose or change doses and trigger times
  • PhysicianSign the chart and approve the plan
  • EmbryologistSpecimen disposition and lab determinations
  • Receiving staffAccept responsibility for a handoff

Hard limits

What OvaFlow never does

  1. NeverPrescribe, or choose or change a dose or trigger time
  2. NeverSign a chart or approve on a clinician's behalf
  3. NeverDiagnose or triage symptoms
  4. NeverRank embryos or predict the chance of success
  5. NeverRelease a lab hold or carry out a physical lab action
  6. NeverTreat a draft or a model extraction as a signature

By role

Who keeps which authority

Each role keeps the authority it already has. OvaFlow shows the work around it.

Authority that stays outside OvaFlow, by role

Handoffs

Handoffs move only on acceptance

Offering work, sending a message or seeing a read receipt never transfers responsibility. Only an explicit accept moves the named scope, and clinical review always stays with the clinician.

In practice

Where the boundary holds in practice

In every scenario, OvaFlow prepares context, checks prerequisites and tracks ownership—while clinical signing, prescribing and physical witnessing stay in the clinic's own systems [1, 2, 3].

How an order moves

What happens in the clinic's EMR versus in OvaFlow

Architectural boundary

Clinic systems of record (EMR, e-prescribing & lab witnessing)

  1. Step 01 · EMRPhysician authors & signs orderDosing, protocol changes, and trigger times are decided and signed inside the clinic's chart.
  2. Step 02 · EMR & PortalNurse verifies in chart & sendsThe coordinator reviews the staged update in the active EMR patient record and publishes.
  3. Step 03 · Lab SystemEmbryologist witnesses sampleSpecimen identity checks, biopsy grading, and cryo movements stay in the lab's own system.

OvaFlow beside the chart (preparation, checks & handoffs)

  1. Beside Step 01Links draft to signed sourceRetires superseded instructions (such as Elena's 20:30 PDT v2) as soon as v3 is signed.
  2. Beside Step 02Blocks wrong-chart placementStops if the open EMR chart (such as Jordan Patel) does not match the patient on the draft.
  3. Beside Step 03Tracks readback & handoffsVerifies the EMR saved the update and keeps follow-up with the sender until accepted.
Walkthrough examples

Who decides and what OvaFlow tracks in four clinic moments

Fictional patient data

Elena Rivera · Evening trigger time amendment (Oct 30)

Qualified staff decision: Dr. Mira Lee reviews the late estradiol result and signs version 3 at 17:10 PDT: Ovidrel 250 mcg at 20:00 PDT on Oct 30 (retrieval Nov 1, 2026 at 07:00 PST).

OvaFlow coordination: Retires the superseded 20:30 PDT version-2 instruction, prepares the version-3 patient update, and tracks Alex Kim, RN's accepted evening callback at 20:28 PDT.

Boundary enforced: OvaFlow never adjusts the trigger minute or marks the order signed from an unsigned note.

Elena Rivera · Post-retrieval OHSS & freeze-all conversion

Qualified staff decision: Dr. Mira Lee cancels the planned fresh transfer and converts Elena's cycle to freeze-all; a named nurse owns Day 1 recovery and follow-up.

OvaFlow coordination: Updates downstream medication instructions, transfer room holds and embryology vitrification plans together while keeping the clinic's 24/7 line and 911 visible.

Boundary enforced: OvaFlow never triages pain or bloating severity or clears a patient for discharge.

Avery & Sam · Two charts and attempt-scoped clearance

Qualified staff decision: Clinicians and embryology staff verify medical, legal and warming clearance for the current transfer attempt in the clinic's systems.

OvaFlow coordination: Keeps Avery's and Sam's medical charts separate and holds readiness when a consent form is scanned under the wrong chart until relocated with an audit trail.

Boundary enforced: OvaFlow never rolls prior-attempt clearance over to a new attempt or performs lab witnessing.

Jordan Patel · Wrong-chart stop & amended PGT report

Qualified staff decision: Nora Ruiz, RN has Jordan Patel's chart open when preparing Elena's trigger instruction; later, a physician reviews Jordan's amended genetics report.

OvaFlow coordination: Stops Elena's draft before anything touches Jordan's chart, and pauses transfer planning on Jordan's amended PGT report until physician review is recorded.

Boundary enforced: OvaFlow never writes to a mismatched chart or releases a laboratory accession hold.

Common questions

Questions clinics ask about clinical authority

Can a nurse coordinator or AI assistant sign a dose change when the physician is in the procedure room?

No. A nurse coordinator can prepare a draft communication from an already-signed order, and an on-call physician can review and sign a new order inside the clinic's own EMR. OvaFlow never allows a draft, a model extraction or a delegated queue item to substitute for a clinician signature.

How does OvaFlow handle authority in embryology and andrology workflows?

Following ASRM embryology and andrology laboratory guidance, physical specimen verification, electronic or dual-person witnessing, biopsy grading and cryo tank movements stay inside the laboratory's own witnessing and inventory systems with qualified lab personnel. OvaFlow coordinates readiness, consents and expected report arrival around the lab.

What happens if a receiving nurse declines or does not answer a handoff offer before shift change?

Until the receiving person clicks accept on that named scope, the task remains in the current owner's queue and escalates along the clinic's time-boxed escalation ladder so it never sits unowned.

Does financial clearance ever unlock clinical readiness automatically?

Never. Front-office prior authorization and medical workup clearance are tracked as parallel columns in the readiness gate. A paid estimate or approved authorization never stands in for missing clinical labs or consents.
